Phone Triage RN & Care Coordinator

Positions Available: 1 Full-Time Temporary with potential for Permanent Employment
Hours: Monday – Friday; 8am – 5pm No nights or Weekends!
Location: Cook, MN

We are currently seeking a Phone Triage RN & Care Coordinator to join our team at our Cook location. This position is primarily responsible for providing telephone triage and nursing services through a dedicated nurse triage line. We are currently experiencing an increased volume in triage calls due to COVID-19 concerns and testing. This position will frequently collaborate with Providers, RNs, Nurse Manager and other nursing personnel. Other responsibilities include care coordination for chronically ill or “high- risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources. The Phone Triage RN & Care Coordinator will operate at the top of their RN license by utilizing advanced clinical nursing skills with discretion and independence. This is a temporary position but has potential to become permanent.

Primary Phone Triage Responsibilities:

  • Assess patient’s medical need and evaluate to determine appropriate care
  • Use written protocols to triage patients
  • Assist and instruct patients regarding their healthcare needs
  • Utilize staff, providers, and nurse manager to make appropriate collaborative decisions regarding patient care
  • Coordinate the required services with other departments as necessary
  • Communicate concerns or problems with the Provider or Nurse Manager for guidance
  • Identify via telephone potential complications and/or need for emergent care
  • Provide patient instructions based on their medical need

Secondary Care Coordination Responsibilities:

  • Serve as contact point, patient advocate, and informational resource for patient’s care team, family/caregiver(s), payers, and community resources
  • Cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals
  • Assist with the identification of “high-risk” patients (the chronically ill and those with special health care needs), and add these to the patient registry (or flag in EMR)
  • Provides mentoring/coaching of other population health and care coordination team members and coach patients/families toward successful self-management of their chronic disease
  • Utilize tools and documents that support a guided care process, collaborate with patients/family toward an effective plan of care
  • Assess patient and family’s unmet health and social needs
  • Provide effective communication to improve health literacy
  • Develop a care plan based on mutual goals with patient, family, and provider’s emergency plan, medical summary, and ongoing action plan, as appropriate
  • Monitor patients adherence to plan of care and progress toward goals in a timely fashion, facilitate changes as needed
  • Create ongoing processes for patients/families to determine and request the level of care coordination team support desire over time
  • Complete Prescription Refill requests
  • Assist support staff with patient inquiries directly or in-directly utilizing phone, patient portal and EMR system
  • Ensure effective tracking of test results, medication management, and adherence to follow-up appointments

RN Requirements:

  • Advanced clinical nursing skills to use discretion and independence while delivering care
  • Excellent Communication Skills
  • Three years clinical experience working with chronic disease patients or related case management experience, five or more years preferred

  • Associate Degree in Registered Nursing with an active MN license
  • EMR or EHR experience
